Our aim at the On Your Game Laser Treatment Clinic Ltd is to deliver a holistic, professional and safe approach to laser treatments. Our Terms and Conditions are there to help you understand how we operate our clinic. Please get in contact with us if you have any questions regarding the below or our services.
By commencing treatment you are deemed to have acknowledged and have agreed to be bound by these Terms & Conditions.
1. The Agreement
This agreement is between the patient (referred to as ‘you’, ‘your’ and “client”) and On Your Game (referred to as ‘On Your Game’, ‘we’ or ‘us’). Where a person as a parent, guardian or attorney arranges treatment on behalf of another under their care, they agree that they will be bound by these Terms. In these circumstances, the references in these Terms to ‘you’ and ‘your’ shall include, as well as the client, the parent, guardian or attorney.
Please read these terms carefully. They provide the basis on which you will be provided goods and services by On Your Game clinic, its staff, contractors and service providers.
On Your Game may update these terms from time to time and you will be notified of any changes that come into effect via one of the following methods:
in person when visiting the clinic
via notifications on our website
via notices within the clinic
via email to the address that we hold for you (it is the client’s responsibility to ensure that the address that we hold for you is correct).
Terms & Conditions will not be posted to you unless this is specifically requested in writing.
2. Rates & payment
Our current standard rates of payment vary per treatment and area being treated. This is subject to change if you have purchased a voucher with a discounted price or a block booking.
2.1. Self-payment
Payment for services is due at the time, or on the day of treatment. We accept cash, card and bank transfer (BACS).
Payment remains your responsibility at all times and where possible you will receive an invoice reminder in the event of non-payment during the session..
2.2. Late payment & outstanding account charges
For accounts outstanding for more than seven days following a clinical session we reserve the right to charge a reasonable administration fee. Failure to settle your account in full or to respond to subsequent invoice reminders may lead to additional administrative charges.
In the case of any funding disagreements between you and your insurer, you remain ultimately responsible for settling your On Your Game account in full within seven days of being notified. You will be required to settle any outstanding amounts in the event of non-payment or part-payment by the insurer for any reason, including due to an excess, undertaking more sessions than authorised, or due to did-not-attend (DNA) appointments or late-cancellations; regardless of the timescales involved.
3. Non-attendance and cancellation
We reserve the right to charge a cancellation fee of 100% of the treatment cost if you fail to attend an appointment without giving prior notice. In the case of cancellation within 48 hours of the start time of the appointment we reserve the right to charge 50% of the treatment cost. Please note that insurance companies may not pay these cancellation charges and you will therefore be invoiced directly.
4. Administrative & additional charges
We reserve the right to charge for any additional administrative time other than that which would reasonably be required to fulfil your appointment booking and payment. Such administrative examples include: clinical reports or copies of clinical records, handling of rejected or late payment fees.
5. Confidentiality & access to records
On Your Game is committed to maintaining strict confidentiality and protecting your personal information. Please refer to the privacy policy on our website for full details on what information we store, how we store it and why. We agree to provide access to your medical records on receipt of a signed and dated medical records access request under the Data Protection Act.
6. Contact details
It is important that you keep us updated of any changes in your contact details. We cannot be held liable should you fail to update us regarding any changes to your contact details including name, mailing address, telephone numbers or email address.
8. Your property
Clinic areas can be busy environments. Whilst we will take all reasonable care to ensure the safety of your belongings, On Your Game does not accept any responsibility for the theft or loss of, or damage to, any of your or your visitors’ property.
9. Offers & promotions
Discounted, promotional and early-bird offers must be paid for in advance and cannot be cancelled, are non-transferable and non-refundable. Discounted offers cannot be used in conjunction with any other offer or discounted package and may be for specific dates/times only. Please also note that discounts are not applicable to patients using medical insurance as a means of payment.

11. Social media

Our Terms & Conditions For Promotions
On Your Game business is otherwise known as ‘Company’ in this document
1. Participation in this promotion is deemed acceptance of the Terms & Conditions,
Terms of Use and Privacy Policy shown on our website.
Promotional Period
2. The promotion starts on 01 Aug 2022 and finishes at close of business on 31 December
2022 unless otherwise extended at Company’s sole discretion.
Promotional Offer
3. “ £29 Initial Laser Consultation ” Sign up online from during the Promotional Period and contact Company to book an appointment. (“Promotional Offer”). It includes a consultation and assessment, any reports, treatments or other offerings are at the sole discretion of the treating practitioner at the time of your appointment.
4. This “Promotional Offer” cannot be used in conjunction with any other Offer and is valid for UK residents and injuries that have not yet been assessed by the Company only.
5. The “Promotional Offer” cannot be exchanged for cash or an alternative gift.

8. To be eligible to claim the “Promotional Offer” individuals must attend a pre-appointment call.

10. This promotion can be claimed on-site only.
11. All claims must be received before 5pm on the end date of this promotion as mentioned in clause 2. Claims received after this date will NOT be eligible for claim
(there will be no exceptions made in this regard).

17. If your claim meets all of the Terms & Conditions you will receive the “Promotional Offer” within 3 weeks. If you have not received it within this time please contact Company with the contact information provided at the bottom of this document.
Eligibility
20. This Promotion is open to all UK residents. Employees and directors of the Promoter, the Promoter’s related companies or any agencies involved in this Promotion, and the immediate families of each of those persons are eligible to participate in this Promotion provided they purchase the “Promotional Product” through the proper channels.

21. Company collects personal information to promote Company products and also to deliver promotional offers, gifts and prizes to participants. All entries received become the property of the Promoter. Please refer to the Promoter’s Privacy Policy here.
Liability
22. Except for any liability that cannot be excluded by law, Company (including its employees and agents) excludes all liability (including negligence), for any personal injury; or any loss or damage (including loss of opportunity); whether direct, indirect,
special or consequential, arising in any way out of the Promotion, including, but not limited to, where arising out of the following: (a) any technical difficulties or equipment malfunction (whether or not under the Promoter’s control); (b) any theft, unauthorized
access or third party interference; (c) any claim or prize claim that is late, lost, altered, damaged or misdirected (whether or not after their receipt by the Promoter) due to any reason beyond the reasonable control of the Promoter; (d) any variation in gift value to

Alter/Conclude
23. Company reserves the right to extend, alter or conclude this Promotion at any time should circumstances dictate. If any participant is found to be abusing this promotion, Company has the right to terminate their claim rights immediately. Should any disputes
arise, the decision of Company is final.
